excuse me if I'm out of date, but arent the drivers you named "betas"?

I thought the latest non-beta driver was 190.62

Oh dear, did this happen as soon as you flashed it or has it been fine until a few days ago and it just started happening for no reason, if thats the case sounds like a memory issue and the card might well be fubarred

nvflash -4 -5 -6 file.rom

were file is your bios name and mi banking on memory error when you get the lines in bios the card is pretty much gone...but if you pray to w/e god you might believe in if any their is a slim..and let me be clear oooo so slim chance of rescuing it

well technically if you flashed it back to the orig bios then yes hey would accept the RMA since its XFX if i remember correctly they have a life time warrentee. However im going to let you decide weather to do it or not because yo flashed the card with a superclocked bios and let it cruise for 2 years...you did something that is not allowed and voids the warrentee so i wont promote ripping off the companys...that decision i will leave entirely upto you.

i know the situation ...
i tried to get back my old bios ! but unfortunatlly the bios version that system shows in gpu-z is not same as the one thats written on the board !
so i have to find that ... maybe my problem will be solved ...

i tested the vga via some kind of vga ram checkin program it was showing me 100000 errors ...

i think the chip is gone ! :D

is there any way to fix it ?! by money or warranty ?!

uninstall all Nvidia drivers using there uninstaller and Driver Sweeper. Reboot, when don't let windows update the drivers, just cancel and get the lastest drivers. Which I believe are 191.07... or something close to that. Reboot again, if the problem still exists. It's you bios. Upload your bios here either me or someone will take a look at it.

* Give us the full detail info on the VGA, Make,Model, RAM and so on. I'll try to find you the orginal

the version should be : 60.80.08.00.37

and the ferqs must be : 576/900 - and for shader 1350 ! as same as this model's Default ...

َASUS ... 768 MB ...

as i said, today i could solve the problem for just a min ... its gone fine like a healthy system but after reset and auto installing of my vga driver by windows it happened ...

Nvlddmkm.sys is crashing because your card is unstable and crashing, just like Windows BSODs when your CPU or memory are unstable/bad. GPUs aren't any different. Your card is definitely toast, and upgrading your drivers isn't going to do anything to fix it. Your only hope is flashing the original BIOS to it before your card completely dies and RMA it, or buy a new, much better card.

If money isn't an issue you should just replace the card. It might be worth it just to avoid the frustration it sounds like you are currently experiencing. If you can't find the proper bios to flash in order to be able to send it in via warranty that is probably your only choice. It sounds pretty messed up and I doubt you are going to be able to "fix" it.

timta2, if im following this right, he has replaced the original card.

and he is now still having the same issue.

i would say the issue is not with the card.

Run memtest.
www.memtest.org

clean out the old windows drivers
http://downloads.guru3d.com/download.php?det=745

go into bios, and remove any voltage modifications to pci express (if you tried to mod the voltage)

and then lastly do not flash anymore bios onto this card.. leave it at stock.

i've had the same error before twice with nvidia cards. Your card is dying as many others have said. I went through the same process as you with my 8800GT's, new drivers, old drivers, different bios's, you name it i tried it but nothing worked. However if you want to make sure you could try downclocking you memory and core by 100mhz or so for good measure and if your errors disappear then yep she's dying on you.
